PICOT Statement
The paper addresses the problem of nursing staff shortage in US healthcare system. The most affected with nursing shortage are Long Term Acute Care (LTAC) patients who are aged 40 years and above and stays for about 4 weeks and four months within the health facilities. The reason for the admission of these patients is mainly due to chronic conditions (CHF, decubitus ulcer that needs wound Vac management and dressing change, ventilator patients, PEG tube feeding, patient who has recent surgery that needs antibiotic therapy and patient who has infection).
Patient/ Population Problem
Nurse Shortage is a significant problem in US healthcare system. The real challenge is to ensure that staffing levels are as per policy, legal and management standards to maintain quality care to the patients (Ferrer et al., 2014). The rise in service demands causes a persistent nursing shortage, a low number of new registered nurses entering practice and retirement of experienced nurses (Coventry, Maslin‐Prothero& Smith, 2015). Another issue that has led to a shortage of nursing staff is the high turnover rate of nurses (Buchan, Duffield & Jordan, 2015). Research shows that one out three nurses leave within two years of beginning to work in a facility. In cases of highly stressful chronic disease nursing experiences, 1 out of 5 new nurses leaves the profession entirely within a year of obtaining practice license (Blitchok, 2018). Indeed, during the situations of nurse shortages, many health facilities find very few nurses who are available for hire and even more difficulty retaining the new hires. In order to cover the nursing shortages, the RNs are employed but they too leave their jobs as a result of turnouts, unsafe working conditions as well as increased workloads(Carnevale, Smith & Gulish, 2015). According to Dunham-Taylor (2014), the issues that are contributing to the turnover of the employed RNs as issues associated with fewer staff members, low salary as well as increased workloads. This issues have been singled out as most problematic with the recruitment and retention of nurses. The high hospital costs, the burdens of retaining nurses and patient related issues that include their safety are indeed apparent. Moreover, Cho et al (2016) indicate that nursing shortage forces existing nurses to work under stressful conditions, which in turn results in job dissatisfaction, injury, and fatigue.Again the authors argue that nurses who are working under these environments are often prone to making mistakes as well as medical mistakes.Consequently, the outcome is that the patient quality can suffer leading to complicati
