Response Plan for Scenario Two: Outpatient chest pain/stroke unit

Concepts of Change Theory

The theory that suits this scenario is the Lewin’s change theory. This theory is the perfect choice because it identifies driving forces as key to leading the people in the desired direction or towards the desired change. Based on this scenario, I would incorporate chest pain/acute MI patients and care givers in identifying the possible solutions to the administration of aspirin and documentation of CT scan. As a manager, my responsibility would be ensuring that these driving forces are enacted in the workplace, which leads the nurses into motivation and become self-driven individuals. The most important thing for the nurses is to conceive in their minds the prerequisites of quality care, especially to conditions such as stroke and chest pains. The nurses should therefore keep good records and adopt a culture that orients them towards achieving the ultimate standards of quality care.

Strategies and Rationale

In order to address the inefficiencies in handling chest pains and stroke patients, one of the most effective leadership style to apply is bureaucratic leadership style. The approach is the best choice because it provides that there must be strict adherence with the governing rules and policies. This means the nurses will have to comply with rules pertaining the administration of drugs, speed for execution of emergency services and so on. The nurses must be guided and bound by specific policies when attending to emergency issues such as caring stroke. In addition, there should be a systematic way of keeping records. which all the nurses must comply with. In examining the processes, I would involve the nurses in various departments as well as the patients, who would give their responses according to their own views. I would consider two groups for accountability purposes and to avoid errors which could result from single team’s information.

Expected Outcome

The outcome of bureaucratic principles can be measured by assessing the nurse’s performance to find out whether they obey the set regulations. For example, if a nurse fails to keep proper records or omits vital information that can lead to future errors and in violation of the rules, such nurse must face a disciplinary action (Hickey and Kritek, 2012). Service delivery can be improved if all the nurses can be able to adhere with the set policies without partiality. This will encompass rules pertaining proper record keeping and offering the patients proper attention.

Professional Standards

The professional standards are important in guiding nurse’s leadership activities. The professional standards guide and promote professional practice. The nurses are therefore able to support their professional development and competence. The professional standards provide a framework which the nurses must comply. Even in dealing with the above situation, the nurses have got define and solve professional practice problems. One of the professional standards that can fully support the above scenario is client-focused provision of service which ensures that the nurses pay maximum attention to the emergency needs of stroke patients.

Response Plan for Scenario Three: School nurse

Concepts of Change Theory

Being a school nurse in an environment where there is high rates of sexual irresponsible behaviors and diseases will require urgent changes of systems that will help in overcoming the aftermaths of sexually irresponsible behaviors. To achieve this, some of principles for change are proposed by Kurt Lewin. The theory identifies three concepts namely, driving force, restraining forces and equilibrium (Disch, 2013). If the driving forces for negative behavior among the students can be identified, then it will also be possible to identify other retraining factors to counter the motivation and urge for negative behavior among the youths. As a nurse I would therefore focus on how to tap the students’ time, attention and availability for profitable activities while at the same time engaging them in programs that enable them to learn about healthy standards.

Strategies and Rationale

In this scenario, the best leadership style to apply is the transformation style. The leadership strategy will help to alleviate the cases of students being involved in irresponsible sexual behaviors that lead to contraction of STDs and early pregnancies (Masters, 2015). According to CDC, healthy students are considered better learners and this is important because it makes the students become conscious of their health life even after school. To address adolescent problems will also call for charismatic leadership style. This type of leadership style is aimed at transforming the team by involving tools such as coaching and mentorship. When this leadership style is applied in school, students will have a perfect forum where they can learn and understand the importance of overcoming sexual desires. The affected students can undergo counselling while others to be nurtured through quality mentorship.

Expected Outcome

Having the students brought together and being taught on the consequences of irresponsible sexual behavior will feasible if charismatic approach is adopted which seeks to attract the students into observing what they learn. Most of the students would be motivated to overcome the trend of negative behavior and prioritize on their health and future of the studies. Through tools such as mentorship, many students would become inspired to pay attention to their career pursuits and not to be entangled by negative practices. On the other hand, counselling the affected students will be taken into account. They need to re-experience a sense of belonging in the community and so they must be helped to overcome the stigma. The success of the applied leadership style can be measured by regularly assessing the response of the students of the learning process.

Professional standards

            As a nurse, various professional standards apply when dealing with the students’ personal issues. One of the things that must be applied is ethical practice which focuses on treating the students fairly with autonomy and without discrimination. Also their private information must be handled with secrecy just as it is required in clinical practice. The professional standards such as honesty, accountability and integrity can be applied in administering healthy behavior among the students and offering any other specialized medical assistance as appropriate.

Response Plan for Scenario Four: Inpatient psych unit

Concepts of Change Theory

In this scenario, the application of institution job rotation theory will help in curbing the current challenges. The operation of the psychiatry department is sensitive and tricky especially when it comes to discharge issues. As a nurse manager, I would definitely put more weight on discharge procedures through tight policies. In ensuring change takes place in the discharge procedures and seclusion of the psychiatry patients, a few principles can be borrowed from Lewin’s change theory. One of those principles can be refreezing which ensures that the systems are renewed or considered to have taken a new orientation. In that case, discharging of inpatients with psychiatry issues will include prior examination which can be done in more than one stage. This is to ensure that the patients qualify for discharging and this must be approved by selected interdisciplinary team.

Strategies and Rationale

As a nursing manager, I would ensure that discharging process involve a comprehensive process of planning and additionally sending the patients back to the primary care physician. In this case, follow-up is one of the critical responsibilities that will be given to the interdisciplinary team. To manage this process with efficiency, the leadership style that is most appropriate in the scenario is Flamholtz and Randle’s Leadership Style Matrix (Smith & Liehr, 2013). This style is best applicable because the interdisciplinary team will have an opportunity to work autonomously according to their specialization. The HBIPS provides that all data must be well monitored to produce the most reliable statistical data.  The interdisciplinary team would involve key hospital stakes such as nurses, physicians and other attendants, community and relatives. This team would collectively determine the need for discharging patients.  

Expected Outcome

The interdisciplinary team will ensure that the patient passes the test, which leads to clearance before being discharged. In addition, they will account the seclusion hours for each patient to ensure a fair time which is within the set time-frame. The leadership style applied gives the team an opportunity to execute their mandate autonomously hence making it easy to measure their participation in the process. In some occasions, improper discharge of patients appears unethical, which can lead to negative publicity of the hospital. To deal with that, decisions must be done with active involvement of all the interdisciplinary teams.

Professional Standards

The professional standards that are likely to apply in this scenario are professional responsibility and accountability and also client-focused provisional service. The professional standards will ensure that the psychiatric patients are handled in the best possible ways and that those isolated are not neglected but they receive a fair service based on accepted professional standards.
