ANSWER

A dashboard is mostly used by hospitals to determine risk factors that may be experienced. This essay seeks to put across three practices that are evidence based and meant to deal with the increased fall rates as seen by provided dashboard.

 Analysis of Data

The provided dashboard shows an improvement in falls. Out of the sampled quarters which are four, three shows falls. NDNQI put across a projection that was not exceeding 3.14 falls within a quarter made up of 1000 days. Firstly, the actual fall as per the units is at 9.19 which is a variable of up to 6.05. The progress was however witnessed in the second quarter. Which had an actual fall rate of no patients as shown within the third quarter. However, it dropped in preceding fourth quarter to a discrepancy of approximately 2.83 with about 5.79 falls. The patients who witnessed falls at a given health care center can be prevented when following certain fall risk routines. I decided to choose the risk for falls as one of the indicators since my work at a rehabilitation center and contend with falls and trying to prevent them oftenly.

Nursing Plan

Dashboards can be said to be ground-breaking methods of examining how a health facility performs through evaluating factors laid down by NDNQI. It was started by The American Nurses Association in the year 1998 and was meant to collect information on unit-based performance and hence compare the state, National as well as regional reports. For the analysis provided, the set database was meant to determine where the strength of a unit as well as its weaknesses as a way of determining the enabling environment for the nurses to lay emphasis on what is deemed best as well as what may need any improvement. Based on proven evidence, using best practices is essential as it changes negative practices to position returns.
