I can also comfortably interpret obtained data and link it with psychopathology knowledge to differentiate between normal and abnormal physiological and psychological changes. In treatment, I am confident enough to apply health promotion and disease prevention techniques. Besides, I can independently evaluate my goals outcomes and modify the plan through reflection of learning experiences. I am also good at maintaining professional boundaries and therapeutic relationships with clients while providing non-judgmental care and still showing empathy. In addition, I have no problem practicing within my scope and completing recommendation documents for psychiatric consultations.

Opportunities for growth: In the process of client assessment and evaluation, I encounter challenges with differentiating between conditions that are physiological from psychopathological ones. For that matter, I will need to read widely and practice more to identify differential diagnoses and formulate appropriate working diagnosis. I will also learn and practice on providing appropriate psychoeducation to clients and relatives for optimal effective treatment. I will also need to seek assistance with identifying situations that present ethical dilemmas and learn on formulating possible solutions.

Also, although I can confidently use assessment tools, I need to train on appropriate selection of screening tools and learn on their interpretation. Finally, I need to practice more on practices I have completed with supervision and be in positon to complete them independently including interpretation of assessment data. I will also familiarize myself more with the DSM V criteria to be able to make correct diagnoses from assessment data and screening instruments results. Now, write three to four (3–4) possible goals and objectives for this practicum experience. Ensure that they follow the SMART Strategy, as described in the Learning Resources.

1. Goal: Over the entire clinical practicum period, I will demonstrate and apply theories learnt in class to conduct a comprehensive psychological assessment to differentiate psychopathological conditions from physiological conditions. a. Objective: Throughout the clinical practice, I will revise my notes and extra readings from books, internet sources and the DSMV criteria on specific clinical manifestations of psychiatric conditions and apply the knowledge during history taking, physical exam and mental status examination to distinguish mental problems from other medical conditions.

b. Objective: Every time I am caring for a client; I will utilize the nursing process to help me link learnt theory with the specific clinical situation. c. Objective: By the end of the practicum period, I will have prepared concept maps for at least five patients with medical and mental comorbidities. 2. Goal: Over the entire practicum period, I will demonstrate knowledge in mental status assessment and measure my knowledge through appropriate interpretation of the data.

a. Objective: I will use the hospital’s mental status assessment tool to gather comprehensive data about client’s mental functioning.

b. Objective: During the first days into the practicum period, I will request one senior nurse to demonstrate interpretation of the information collected in order to generate both clinical and nursing diagnoses. c. Objective: At the end of every assessment session, I will interpret information collected via mental status assessment tool and generate appropriate working nursing and clinical diagnoses.

3. Goal: By the end of the of the practicum period, I will have provided psychotherapy to at least ten individual clients and conduct at least five group therapies. I will follow-up the clients to evaluate the effectiveness of the psychotherapy. a. Objective: Before conducting any individual or group therapy, I will identify the client’s problem first and read about necessary patient education prior to the actual session. b. Objective: I will organize a quite environment or room with adequate lighting and ventilation and provide comfortable seats for the clients to promote comfort throughout the period.

c. Objective: For group therapies, I will organize clients with similar condition or who need similar psychotherapy training for easy demonstration and avoid client’s feeling out of place as others take part. 4. Goal: Throughout the entire practicum period, I will demonstrate and apply effective communication skills in order to establish therapeutic interpersonal relationship with patients having mental problems and with other health care workers. a. Objective: I will always actively listen to client’s concerns and respond with respect and at timely basis. b. Objective: I will use appropriate facial expressions, gestures, touch and upright posture when conducting psychotherapy to establish a good communicative relationship with clients. c.
