Patient safety is the foundation of all patient care. Safety is critically important in all
healthcare organizations. With the growing populations of healthcare, safety is vital. Educating
healthcare staff is always an important topic that can never be an ending subject. The goal is to
provide education in leadership so all staff has the skills to be successful. Healthcare is not as
safe as it should be. Medical errors in the healthcare is the leading cause of death and harm to
patients.
As a registered Nurse of more than 15 years, I’m responsible for hands-on care of patients. I
am directly involved in treating patients by administering medications, obtaining lab draws,
administering blood products, managing IV’s, monitoring patients’ conditions, accurate charting
and communicating with all team members. Nurses that are rushed tend to make medical errors.
For example, one of my colleagues obtained the wrong blood administration from a patient for a
type and cross. This error reached the wrong patient and blood was administered and caused
death to the patient. This error implemented protocols in play to have two nurse verification
obtained when drawing blood. I implemented a plan to obtain two type and cross lab draws 5
minutes apart and sent to the lab at 5-minute intervals. This incident was helped the staff realize

I confirmed that the articles were from healthcare articles and physicians. The article on
admitting mistakes was from credible physicians. I feel this was defined in what I was wanting
to include. I was wanting to ensure all information was related to healthcare and patient safety.
