To care for someone, I must be able to bridge the gap between myself and the other: I entered into the nursing profession because I wanted to help people and to make a difference in someone’s life. Before I could truly accomplish this I had to look at my own beliefs and the cultures and beliefs of the patients I care for..  Patient’s beliefs  play an important role in the delivery of their care.  In the  nursing profession, personal presence dictates my decisions and actions , as it is the foundation of what I live by.  Nursing presence demonstrates caring, empathy, and connection, qualities required to build rapport and trust between nurse and patient. (Boeck, 2014).  

Professional presence   leads towards the trustworthy relationships between the health care professionals  the patients, their families and  the community.  Throughout my nursing career, the profession has made significant progress both socially and technologically, this includes the nurses understanding of health and healing as evidenced by Dossey’s three eras of medicine. My own past experiences with life and illness impact my delivery of care to patients and families; this course has helped me discover my own beliefs and how that effects my daily work environment.

A1: Compare Two Models of Health and Healing

Era 1, (body) beginning in the 1800’s, reflected the fundamental view that health and illness are totally physical in nature, all about the body. (Dossey, 1999)  The effect of the mind or soul did not enter into treatment in ERA 1.  In ERA 1 , the body was nothing more than a machine, as parts broke down they were  removed or fixed.  Era 3 (mind,body , soul) began in the 1990’s and is still evolving today and builds on the prior era’s.  In Era 3, the mind is boundless, and may not only effect a person body, but the body of another person (Dossey, 1999)

A2: Analyze the Difference of ERA 1 and ERA 3

Over the years the Era’s of medicine have changed, the connection between the eras are intertwined like a braided rope, yet very different as evidenced by Era 1 and Era 3. The  main focus in ERA 1 is on the body and addresses the health and healing of humans based on bodily functions and how it can be treated.  (Dossey, 1999) The body in ERA 1 is nothing more than a machine with parts that breakdown like a car.  Era 3 takes a more holistic approach by incorporating the body, mind and spirit, the body is not just parts as discussed inWatson’s Theory of Nursing, which states that human caring consciousness, administering ‘human care essentials’, which potentiate alignment of mind body spirit, wholeness, and unity of being in all aspects of care, tending to both embodied spirit and evolving spiritual emergence (Falk-Rafael, 2005). 

My nursing career spans 36 year, which has included clinical areas such as the ED, ICU and now in an administrative role focusing on quality outcomes and  patient experience .  Medicine has significantly progressed since ERA 1, in the early twentieth century most medicines prescribed were prepared by the pharmacist.  In 1928, Fleming discovered penicillin which changed the treatment of care from how surgery was performed to how wounds were treated.  In ERA 1  mental illness was treated by preforming a prefrontal lobotomy or shock therapy.

My nursing career mainly evolved in ERA 3.  Over the past several years the fundamentals of ERA 3 intrigue me; The World Health Organization states that “it is a state of complete physical, mental, and social well-being, and not merely the absence of disease or infirmity and is a conscious, self-directed process” (WHO, 2013) Era 3 is more of an era of integrated medicine ,this approach  blends conventional medicine, including use of drugs and high-tech techniques, with evidence-based, natural, less invasive therapies examples include dietary change, meditation, acupuncture, yoga, herbal medicine and therapeutic massage  (Freudberg, 2013).  The adoption of the mind,body, spirit model  has proved to be an effective method to promote well-being.

Examples of this includes the practice of Lamaze during labor, ,the use of aromatheraphy to help decrease anxiety and deceasing the use of opioids is a whole new era for nurses and physicians.  Hospital administrators understand the importance of ERA 3 and have now employed full time chaplins to pray with patients and not just give last rights.  The integration of the mind body spirit model into  treatment has significantly enhanced  mental and physical health along with the improved  sense and support from the social entities.  

  My recent work involvement was in the development of a policy on M technique therapy. This is  a registered method of gentle touch, it is suitable for the critically ill, actively dying, very fragile, or anxious patient.  The M-Technique has been described as ‘physical hypnotherapy’, ‘a spiritual dance’ and ‘the essence of nursing’ and is profoundly relaxing for both giver and receiver (Buckle, 2008).

In the nursing profession, the concepts of ‘presence’ and mindfulness have gained immense popularity.  Awareness of one’s personal beliefs, how it affects work situations and how it  may potentially affect the way a nurse views a patient, or a patient’s condition and care is paramount (Mendes, 2015).  In the early nineteen eighty’s  the AIDS epidemic and recently with the increased heroin addiction is an example of how personal beliefs affect work situations.  The real key is to find solutions to discrepancies between personal values, patient beliefs and our professional duty to care for patients and families without burnout.   I am a very mindful nurse, especially as a patient advocate.  I am able to critically think before making a decision, because of my years of experience I am confident enough  to make decisions in emergency situations.  McCormack and McCance (2010) developed the original Patient Centered Nursing Framework. The framework defines five attributes as fundamental to patient centered nursing and they are: being professionally competent, having developed interpersonal skills, commitment to the job , clarity of beliefs and knowing self (McCormack & McCance, 2010).   

 Professionally my work ethic is a  mixture of ERA 1  and ERA 3.  As the Director of Nursing Quality, one area of developing high quality is on concentrating on stabilizing the body by improving door to needle times with t-PA for the ischemic stroke patient but as the patient advocate,  consideration for a patient’s emotional, spiritual and cultural factors is essential because of the influence on the patient experience.  For example, a  few years ago  my assignment was to care for an Asian women with breast cancer and diabeties,and her appetite was very poor.  Her culture included white rice with every meal, and her belief was that she should eat only fresh organic fruits and vegetables  to promote healing and prevent further disease progression.  She also practiced daily meditation and prayer. Her  order diet was a standard diabetic diet,   was placed in a double bedded room,  which did not allow for privacy, meditation or prayer.  As the days went on she became increasingly depressed because her cultural beliefs  and needs were not considered.  By focusing on her physical body needs of chemotherapy and diabetic management, the interdisiciplinary team did not focus on  her mind body spiritual needs, one could argue this caused more harm to the patient. The practice of professional presence and mindfulness allowed me to listen to the patient with patience to effectively develop a multi disciplary plan toward  the intervention.  Eventually the team was able to move her into a private room and provide  small separate refrigerator in her room for her own foods. Keirsey Temperament Personality Test

            The Keirsey Temperament Personality Test is the most extensively used questionnaire for the self-assessment which is designed with the aim to help the individuals in understanding themselves as well as the others. The test describes the four different temperaments, i.e. Artisan or iconic, Guardian or Pistic, Idealist or Noetic and Rational or Dianoetic.

            Artisans involve the adaptability and tangibility of a person. Such individuals seek for the virtuosity and stimulation with the greater concern to induce the impact on others. The biggest strength of such individuals is tactic. Artisans have the excellent grasping power and agility, troubleshooting and manipulation of the instruments, tools and other equipment. The person can either be an operator or proactive, or an entertainer or reactive individual.

The Guardians, on the other hand, are the scheduled, organized and concrete persons. Guardians seek for belonging and security with a  major concern towards their duties and responsibilities. Logistics is their biggest strength, because Guardians excel in the facilitation, organization, supporting and checking tasks. Such individuals can be either an administrative person or a conservator. Guardians are the corner stones of society. (Keirsey, n.d.)

In addition, the Idealists are the compassionates and abstractors, where they seek  the significance and meaning of their  profession. The Idealists major concern is their  growth at a  personal level with the identification of their own distinctive identity. Where the diplomacy is the biggest strength of such personalities. Idealists generally excel in the unifying, inspiring, clarifying and individualizing, with the major role as the mentors and advocates.

Rationales are the personalities who are objective and abstract oriented. Rationales seek for the self-control and mastery, showing main concern with their personal competence and knowledge. Strategy is their remarkable strength, whereas they excel in the conceptualizing, theorizing, engineering and coordinating. Thus, Rationales play a major role as the engineers or coordinators (Keirsey, n.d.)

Test Result Analysis

My personal results for the Keirsey Temperament Personality Test revealed my type of temperament is   the  Guardian. There are four types of Guardians: Supervisors, Inspectors, Providers, and protectors. This type of personality perfectly aligns with my personal traits; for instance, I am a practical person, who can make decisions quick and am also very goal oriented. I believe in certain core values, among them the belief in a strong work ethic, the need for people and institutions to be responsible and accountable for one’s actions, the importance of following the rules  respecting  authority and traditions.  I have lot of company as a Guardians , In my organization the majority of Nursing Administarion are Guardians.  As a teenager growing up I never thought of breaking curfew, in addition, family traditions are very sacred to me.  Birthdays, anniversaries, holidays  events like these seem to hold a special value to me because this bring those I love and care about together.  One of my greatest attributes is being trustworthy and dependable. My family and friends can count on me for anyting, as well as my Chief Nursing Officer (CNO). My reliability at home and at work is evident through my hard work and responsibility.   Keirsey considers the Guardian to be  helpful and dutiful,  this is  why over the years I have been asked to be a chair several committees and community projects.  Possessing the personality traits such as down to earth, humble yet practical, I feel comfortable with new  happenings going on around me,I am able to efficiently complete projects before starting   new ones. I strongly believe that my loyalty at work, ability to lead or supervise a team, and discipline can effectively lead me and my team members to the right path of success. As a leader, my success is not only measured by how successful I am in getting my teams to reach their goals, but also by how I attract and keep followers.  On a personal level, the test is very accurate I have two or three friends in my inner circle, and definitely prefer small intimate affairs where I am not the center of attention.   As a Guardian  I have the ability to manage circumstances and behaviors, ranging from my duties in Nursing Administration  to bedside nursing.  On another hand, I have to disagree with some the personalities described by the Keirsey Temperament Test. Challenges

There can be various potential barriers that may arise during the course of practicing my profession, in particular, while dealing with the individuals possessing opposite personality traits, especially the rationale personality.  For instance, the likely challenges or barriers can be in cultural competency, and the lack of need to get the job done timely. My colleague who  happens to be a Rationale personality is always looking for a better shorter way to get thing done.  These  individuals may show resistance for the  rules and regulations,  I tend to take things too personally and my work  suffers when the personal relationships with colleagues are not harmonious. Another barrier that is often a challenge is workplace communication, often Rationale peers beat around the bush, I on the other hand can be quite direct in my communication, which may have a tendency to offend some co-workers.

With the recognition of these  personality traits generally I tend to show positive emotional behavior towards others with opposite personalities during the situations they feel offended. Instead of criticizing, and being confrontational or showing negative emotions, I believe, I try to behave optimistically, cooperatively, respectfully and politely. I used to show very few verbal expressions previously, but with the awareness I try to communicate with others and plan for short term goals in order to remove the communication barrier. This is extremely important to my work as a Patient Advocate, the interactive and interpersonal relationships significantly affect the outcome, while a major focus must be towards  restraining my feelings of frustration and anger concerning others.  In addition to this, I value that an effective  manager needs a unique set  of qualities, such as courage, integrity, ability to cope with stress and inventiveness. I  appreciate others for their efforts in critical thinking, enhanced collaboration and communication, and for clear definition of goals. Being a good Guardian manager ,I encourage my staff to work in an organized manner following the rules to achieve the strategic goals set by my organization to improve patient care.

Spiritual Goal

The psychological and spiritual wellness of the person assists  in various ways.  In order to develop the required state of health and wellness, an individual must perform  spiritual acts and exercises.   Being a nursing professional, I try to understand all of the personality dimensions of a patient to  fulfill their needs.  In order to assist the individuals in attaining the spirituality, it is necessary to attain it first for my own self.  The personal goals would include the strengthening of my own spiritual powers. Being raised and brought up in the Catholic faith, I have a strong belief in traditions and norms. Physical Goal

I would like to improve my physical health which will not only benefit me in physical terms such as control of heart rate, blood pressure and cholesterol, but also would improve my symptoms of Rheumatoid Arthritis.  Over the last year I have become more sedentary.  One of my physical goals is to return to my healthy eating life style of the Paleo Diet, which has anti – inflammatory benefits. Since much of my day is spent in meetings, I plan to make a conscious effort to walk each day at lunch for thirty minutes.

Emotional Goals         

 The state of being emotional refers to relating to feelings and the way they affect your life. (MacMillian, 2016)   I aim to challenge myself emotionally, as it is necessary  to combat the negativities related to certain surrounding circumstances.  I plan to accomplish this by the discussing the situations or feelings I am having immediately with my family, or co-workers and not having these feeling bottle up, this  would reduce the stress levels of my very fast paced personality. I also intend to ignore the frustrations due to time deficiency (Seaward, 2012). This would help me in overcoming the stressful situation during  practice of the profession.  My plan is to keep a daily journal, and learn to meditate.

Vital or Rhythmic Goals

It is necessary to maintain a  balance between  family, relatives and this stressful profession.  Personally, I need to create and maintain this healthy balance in order to be an effective role model for the patients as well as my family.  I aim to maintain the cardiac rhythm within me. A panicked  and distressed individual is of no help to anybody, either patient or a family member. I need to have control of my own nerves which starts with my sleep cycle. Achieving the Goals

Physically, I consider myself as a strong and spirited person, where for the very few times I need support from others, however, I will need others support and help of my family and friends to achieve my  goals in an effective manner. To maintain the weight, healthy exercises, I would convince others in my neighbors or friends to join and support me.  For example with the support of two colleagues I now go to the gym at least three times a week and I joined a group of coworkers who walk daily at lunch.

My rhythmic or vital goals would require the balance between my daily routine activities. Since, my personality traits follow the Guardian, I would utilize all my strengths to draft a schedule in my outlook calendar.  Maintenance  of the sleep cycle and relaxation is also a critical factor. The recliner cannot be an option after dinner or chores, usually I fall asleep there for a few hours and then after much encouragement by my husband, I  go upstairs to bed.  I will schedule a housekeeping chore nightly like cleaning out a cabinet draw, instead of watching television in the recliner.  I also plan to set the alarm on my phone for nine and go straight up to bed.   By this, I would be able to establish the circadian rhythm, which would lead to wellbeing of the whole body.

As stated earlier, my spiritual wellness can be attained  during this upcoming Lenten season, by committing to visit my religious institution with family and friends. Since my grandson is making his communion this upcoming year, I will accompany him to church each Sunday and teach him to pray the rosary.  While teaching him the rosary, I will also be attaining my goal of praying the rosary weekly. With the active participation in the activities performed in church,. this would build a strong and healthy foundation for moral values not only in my own self but also in others.

Emotional goals will be the most challenging for me to achieve, one way I plan to succeed with this is to download the application  CALM on to my smart phone, this is a mediation application for beginners.  On  the days that I go to the gym, I finish with three minutes in the sauna, during this time I plan to listen to the app.  I also have purchased a daily journal book that I will leave  on my pillow after I make my bed in the morning. By keeping the journal on my pillow , this will remind me to  write about my feelings before I go to bed.

Since I fear failure,  I would continue challenging my emotion and mental capacities, by pursuing and  attaining the professional degree that I am working on, this would help me to incorporate the best evidence based practice (EBP) in my own personal practice. Meeting my goals will allow me to expand my mind, body and soul, I have learned a long time ago , I am of no help to my family, patients, or coworkers ,if I do not care for my mind body and soul.

The Samueli Institute is a nonprofit research organization dedicated to an Optimal Healing Environment (OHE), this environment is one that supports and stimulates patient healing by addressing the social, psychological, physical spiritual, and behavioral components of healthcare (Samueli, 2015)  The OHE  framework is used by healthcare organizations to help identify  any deficiencies and improve outcomes within their foundations of care.  The framework consists of four domains for improvement: Internal, Intrapersonal, Behavioral, and External. To sum it up optimal healing means to focus on an individual as a whole, either  it can be a patient , family or staff members. In reviewing the literature two such facilities stand out  Cape Coral hospital in Florida and Plantree, Good Samaritan Hospital..  Cape Coral Hospital strives to serve as the community’s premier hospital and center for health and wellness by providing an Optimal Healing Environment focused on health, prevention, healing and connectivity. (Cape Coral Hospital, 2016) They have created a comfortable healing atmosphere that patients can utilize such services as Arts In Healthcare , a nation-wide movement that links the expressive arts with the healing, family birthing suites, and a community diabetic education program .  At Cape Coral Hospital is a smoke free campus, which also offer healthy choice lunches and healing rooms for their staff.

Good Samaritan in Nebraska is not only a Plantree designated faculty it is also a Magnet facility.  Like cape coral they offer art therapy and birthing suites, however they have taken it a step further. Their hallways are filled with fish tanks and low lighting and several healing gardens Their mission is, “To nurture the healing ministry of the Church, supported by education and research. Fidelity to the Gospel urges us to emphasize human dignity and social justice as we create healthier communities.” (CHI Health, 2016). As a Magnet model their nurses follow the guiding principles of patient centered care and staff centered work environments well as supporting the valued balance between work and family life. In our community, my organization facility is one of three in a seven mile radius. Internally, our senior administration has elevated the organization’s standards by  offering “ Meatless Monday”, removing all sugary drinks from our cafeteria and opening up our physical therapy department after hours to promote exercise and a healthy life style.  Considering the effective awareness about my profession and its requirements, I intend to strengthen the patient advocacy position in my facility by helping  in making decisions regarding the value of health and wellbeing along with patient satisfaction.  In viewing other Healing Environment facilities like Cape Coral Hospital in Florida, and Good Samaritan in Nebraska ,we are very similar.  All three health care centers are keenly putting efforts to promote the healthy lifestyle and care within the community.

Effective recognition of my personal and professional skills and the acknowledgment to the situation enables me to tailor many situations.   Nurse theorist Virginia Henderson, states “the unique function of the nurse is to assist the individual, sick or well, in the performance of those activities contributing to health or its recovery that he would perform unaided if he had the necessary strength, will or knowledge. (Henderson, 2016).  The identification of my own personality will effectively allow me to improve the quality of care for my patients. With the identification of these personal characteristics  , this allows me develop and improve the required skills for the better performance as a nurse leader.
